Third party (person) request
  1. Handwritten request (in Ukrainian or Russian) addressed to the Rector of KNUTE Anatolii Mazaraki. Has to have in it:
     - request to grant needed information;
     - № of diploma and issue date.
  2. Scanned diploma and supplement of diploma.

2. Consent for processing and granting personal information to the third party.

3. Scanned diploma and supplement of diploma.

All the abovementioned documents have to be scanned and sent to the KNUTE e-mail knute@knute.edu.ua or hard-copied by post on address: Kioto str. 19, Kyiv 02156, Ukraine.
